THE COMPREHENSIVE INITIAL ASSESSMENT TOP
Each tool is a comprehensive assessment designed to aid the clinician in initial assessment and treatment planning. ADULT & CHILD TOP SYMPTOM SCALES
This questionnaire covers all of the clinical symptom scales of the TOP INITIAL (The Symptom Scales do not include case-mix and functional questions). It is an excellent tool for obtaining an overall analysis of patient progress over time. The domains include:

The TOP SA is designed for use with patients with substance abuse or dependence issues. The TOP SA is patterned after the ASI, but is quicker and easier to administer. THE COMPREHENSIVE FOLLOW-UP ASSESSMENT

This tool is similar to the INITIAL and includes all the clinical and functioning scales. The third page of demographic questions is replaced with a satisfaction questionnaire.
